Connecting to Resources
Our Mental Health - Targeted Case Managers support children, their families, and adults with a mental health diagnosis in connecting to resources and providers within their home community. Targeted Case Managers complete person-centered functional assessments and plans in collaboration with the individual and/or family, make valuable referrals, monitor service provision, and coordinate overall mental health care. This “wrap-around” service can complement existing counseling or therapy services provided by our staff, or be offered as a stand-alone service.
