Providing Services for Youth in the Schools

Our School and Family Mental Health Program is an early intervention program for students with a mental health diagnosis to support and promote the social, emotional, and mental well-being of school-aged children within the school setting. The goal is to help youth feel more competent, confident, and successful in school, at home, and with peers.  Referrals can be made by contacting the counselor or social worker in your school. 

 

Referrals are given by:

  • Parent
  • Teacher
  • School Administrator
  • Community Service Provider 


Our Program Provides:

  • Diagnostic Assessment and Treatment Planning
  • Individual and Group Counseling
  • Family sessions
  • Therapy and CTSS skills training that focuses on healthy coping skills, managing emotions and symptoms, conflict resolution, and other social skills
  • Problem solving and support for anxiety, school behaviors, grief, family change, crisis planning, and other concerns.
  • Resources & information on parenting issues, school concerns, and mental health information
  • Summer programming with social, emotional, and educational activities
  • Community resource and referral information
  • Mental Health Targeted Case Management (MH-TCM).

Helping Connect in the Community

TARGETED CASE MANAGEMENT supports children, families, and adults with a mental health diagnosis by connecting them to resources and providers within their home community. Our  Targeted Case Managers complete person-centered functional assessments, make valuable referrals, monitor service provision, and coordinate overall mental health care. This “wrap-around” approach can complement existing counseling or therapy services provided by our staff, or be offered as a stand-alone service.

REACH

Partnering with Hutchinson Public Schools

The REACH Program is a voluntary, in school program designed to assist students who may need support academically, socially, and/or emotionally. REACH partners with Hutchinson Public Schools to help students experience success by learning new life skills to overcome barriers and/or challenges present in their lives.

Though the needs of each student varies, on average a student may benefit from services from anywhere between 3 months to 2 years.


Depending on the need, average sessions may last anywhere between 30-90 minutes.


The SFMH program utilizes a variety of tools to assess each student’s progress.  The SFMH provider regularly consults with a team of mental health practitioners and professionals  to discuss each case. Additionally, every 6 months, the counselor and family meet with a qualified mental health professional to review the outlined goals and determine the progress made. The SFMH counselor also administers two methods of evaluation in 6 month increments (the CASII and CBCL) as requested by the Department of Human Services (State of MN). 


YES!  These services are confidential.  However, with written parent permission, progress and goals may be shared with the school or other associated community programs.
